How to fill out Part B of Form T2201
The Canada Revenue Agency (CRA) makes a decision about your patient's eligibility for the disability tax credit (DTC) based on the information given on Part B of Form T2201.
Before you start
Before you start, make sure you can fill out the applicable section for your patient.
Medical practitioner | Can certify |

To complete the process, you will need the following information:
- your patient's name and date of birth
- details about your patient's impairment(s): assessments, medical records, or any other documentation that will assist you with answering the questions during this process
Use your best professional judgment when filling out Part B of Form T2201 for your patient. Make your assessments based on the following information:
- the symptoms reported by the patient
- your knowledge of the patient's medical history
- your knowledge of effects of the impairment(s) as they relate to your patient's medical condition
Filling out Part B of Form T2201
There are two ways to fill out Part B of Form T2201:
Digital application
Step 1 – Use the digital application
Access the new digital application for medical practitioners. The application will take you through several steps to collect the following:
- basic information about you
- your patient’s name, date of birth, and which impairment categories apply to them
- information about your patient’s impairment(s) such as diagnosis, effects of the impairment, duration of the impairment, and potential for improvement
- if your patient is potentially eligible under the "Cumulative effect of significant limitations" category, you will be asked to answer a few additional questions relating to their eligibility under that category
Step 2 – Print and sign the PDF
Once you've completed the application, you will generate the PDF that will be submitted to us to determine your patient's eligibility. After printing, be sure to sign the Certification section on the last page.
The system does not save a copy of your responses or generated PDF. To keep a copy for your records, you can save the PDF to your computer, print an additional copy, or make a copy after printing.
Step 3 – Give the form to your patient
Once signed, you will need to give Form T2201 to your patient for them to complete Part A and submit the form to us.

Step 1 – Get Form T2201
Go to Form T2201, Disability Tax Credit Certificate, for the most recent version of the form. You can either use the fillable PDF to fill in your responses prior to printing or print the form to complete it by hand.
Step 2 – Fill out the applicable sections for your patient
Part B of Form T2201 begins on page 3. Pages 1 and 2 constitute Part A, which must be filled out by your patient or their legal representative. Each impairment category has its own self-contained section on Form T2201. Fill out only the section(s) that apply to your patient and make sure you answer all the questions, as applicable.
If you fill out the "Cumulative effect of significant limitations" section on page 14, make sure you also complete the individual sections for the impairments that contribute to the cumulative effect.

Step 3 – Fill out the Certification section
Answer the questions and provide your contact information. You must also certify that the information in Part B is correct and complete. You must sign the form.
You or your patient can send Form T2201 to your patient's tax centre. If you need to send us a letter, you can send it to the same address.
After the form is submitted
If the information provided is insufficient or contradictory, we will contact you in writing for clarification.
We will review the information you provide to determine your patient's eligibility for the DTC. We will advise your patient of our decision concerning their eligibility.
