Self-assessment questionnaire

Answer

You may be eligible for the DTC.

Take Form T2201, Disability Tax Credit Certificate, to a medical practitioner who can certify the effects of your impairment for you. If the medical practitioner certifies the form, send it to us for approval. We will review your form and advise you in writing if you are eligible for the DTC.

If you are eligible for the DTC, you may be able to claim the disability amount. For more information, see Claiming the disability amount.

You may also have expenses that you can claim on your tax return. For more information, see Guide RC4064, Disability-Related Information, or use the following links:

You have told us:

  • your impairment has lasted, or is expected to last, for a continuous period of at least 12 months
  • your impairment causes you to have limitations in one or more of the DTC categories
  • the effects of your impairment cause you to have a marked restriction all or substantially all of the time (at least 90% of the time) in one or more of the DTC categories, even with appropriate therapy, devices, and medication
