T4A(P) Statement of Canada Pension Plan Benefits

Tax Information Slips Online Service
With Service Canada's Tax information slips online service, you can access your T4A(P) slip earlier, and file your income tax return sooner.
Box 20 - Taxable CPP benefits
Enter this amount on line 114 of your return. Box 20 includes any benefits shown in boxes 14, 15, 16, 17, 18, and 19. It also includes any recovery of Canada Pension Plan overpayments or payments for arrears.
Box 21 - Number of months - disability
Number of months excluded from your CPP contributory period due to disability.
Box 22 - Income tax deducted
Enter this amount (dollars and cents) on line 437 of your return.
Box 13 - Onset or effective date
For a disability benefit, this is the date Service Canada determined that you were disabled for Canada Pension Plan purposes.
For a retirement benefit, this is the date the benefit became payable.
Box 14 - Retirement benefit
This amount is included in the amount shown in box 20.
Box 15 - Survivor benefit
This amount is included in the amount shown in box 20.

Box 16 - Disability benefit
Enter this amount on line 152 of your return. Line 152 is located below and to the left of line 114. This amount is included in the amount shown in box 20.

Box 17 - Child benefit
This amount is included in the amount shown in box 20.

Box 18 - Death benefit
This amount is included in the amount shown in box 20.

Box 19 - Post-retirement benefit
This amount is included in the amount shown in box 20.
Box 23 - Number of months - retirement
The entry in box 23 represents the number of months in the year that you received Canada Pension Plan retirement benefits.
If you are an individual who is 60 to 70 years of age read, Canada Pension Plan (CPP) contributions for CPP working beneficiaries.
