Affidavit form for intestate situations
If you are a family member of the deceased who died intestate (no will), you can complete an affidavit form to be recognized by the Canada Revenue Agency (CRA) as the person willing to manage the tax matters of the deceased.
Select the Affidavit form in accordance with the deceased’s province or territory of residence.

Follow the instructions attached to the Affidavit form and mail the completed form with the requested documents to the deceased’s tax centre.

Note
If the deceased was a resident of the province of Quebec, you need to go to the Revenu Québec website and complete form BD-81.7-V (2013-09) and follow their instructions.
