Registered Charity Basic Information sheet (TF725)

Form TF725, Registered Charity Basic Information, is system-generated and included with the T3010 annual information return package. We mail it to you in the month after the end of your fiscal period.

The information on the basic information sheet and Form T3010 is available on the List of charities. Review the information to make sure it is correct.

Attach the basic information sheet to your return when you file it. The person who signs the return is certifying that the information on the sheet is correct and complete.

How to correct the basic information sheet

To correct any of the pre-printed information in the following list, make the changes directly on the sheet in the boxes provided:

  • mailing address
  • telephone number
  • fax number
  • email address
  • website address
  • public contact name or position
  • name your organization is known by other than its registered name
  • program areas (see program areas and field codes)

To correct the pre-printed information in the following list, send us a letter, signed by an authorized representative, along with supporting documents:

You cannot change the registration number or registration date shown on the sheet.

The basic information sheet might also include a message to confirm if any of the following applies to your organization:

Contact the Charities Directorate in the following situations:

  • you previously asked for changes that do not appear on your sheet
  • you lost your sheet
  • you never received your sheet

Peel-off labels

The basic information sheet includes peel-off bar code labels. Attach them to each of the following that applies to you:

  • the envelope or package for sending the return

  • Form T3010, Registered Charity Information Return

  • Form T1235, Directors/Trustees and Like Officials Worksheet

  • Form RC232-WS, Director/Officer Worksheet and Ontario Corporations; Information Act Annual Return, or Form RC232, Ontario Corporations Information Act Annual Return

  • Form T1236, Qualified Donees Worksheet / Amounts Provided to Other Organizations

  • Form T2081, Excess Corporate Holdings Worksheet for Private Foundations

  • financial statements

  • separate sheets used to complete the tables in the return
