Section D – Other promoter information
Promoter's name, identification number, address, and telephone number – Enter the name, identification number, complete address, and daytime telephone number for each promoter of this tax shelter, including any broker being used to promote or market the product. Follow the instructions for Complete address.

Type of promoter – In the last column, enter the code to identify the type of promoter.

| 1 | for an individual, other than a trust |
| 2 | for a trust |
| 3 | for a corporation |
| 4 | for a partnership |
| 5 | for a nominee or agent that holds an interest in the partnership as nominee or agent for another person |
