Completing the T4 slip for revocations and what to do with Form CPT30
You should complete the employee’s T4 slip in the usual way. However, you must make sure that:
- the CPP contributions (Box 16) and the CPP pensionable earnings (Box 26) agree with the employee’s payroll records from the revocation effective date to December 31
- the “CPP/QPP exempt” box (Box 28) is left blank
After we receive and process the T4 slips, we will check the calculations for any CPP deficiencies during our pensionable and insurable earnings review. We will advise you if our calculations do not match what you reported. For more information, go to CPP deficiency calculations.
What to do with Form CPT30
You should keep a copy of all completed CPT30 forms with your payroll records. For more information, go to Keeping records.
