Pre-claim consultation request form
To ask for a pre-claim consultation, complete the form below and send it to the Canada Revenue Agency (CRA).
Instructions
1. Select the tab for the tax services office (TSO) that serves your business's location
(For example, if your business is located in Laval, select the "Western Quebec Tax Services Office" tab. The locations served by each TSO are indicated under each tab.)
2. Fill out the form and click on "Submit"
