This form is used to calculate the portion of the yearly basic exemption and the maximum contributory earnings to be applied to the CPP and QPP. Complete this form if you earned employment income in the province of Quebec and were a resident of a province or territory other than Quebec or if you earned employment income in a province or territory other than Quebec and were a resident of Quebec.
