Business and project information required

After you send your completed form, a Canada Revenue Agency officer will call to discuss if you qualify for this service.

To prepare for your call with the officer, you will need to have information on hand for each of the items below.

Part 1 – Business information

Business name, which has to be the same as the name used on your income tax return

Business number or social insurance number

Business address

Business tax year

Date of your request

A contact person for financial information; their position; and phone number

A contact person for technical information; their position; and phone number

For how many projects do you want a consultation? (You can have a maximum of three.)

Part 2 – Project information

The date of your consultation request must be no later than four months after the end of the tax year in which the projects started

Each project in your request must meet the following requirements:

  • is completed or in progress in the tax year
  • was not already reviewed and given an eligibility determination
  • does not involve an issue that is under notice of objection or in litigation

For each project that meets the above criteria, give information as follows:

Project name

Brief project explanation

This must include information on the field of science or technology in which your business is active; the challenging scientific/technological problem you faced; the work you did in the tax year to try to solve the problem; and how the results of the work you did represent an advancement in your understanding of the related science or technology.
