Proof of birth

Acceptable proof of birth can be a photocopy of any of the following documents, on which the child’s family name, given name and date of birth appear:

  • a birth certificate;
  • a birth registration form certified by the registrar of a province or territory;
  • the hospital record of birth;
  • the record of the attending physician at the birth;
  • the record of the attending nurse or midwife at the birth;
  • a record of landing or permanent resident card issued by Citizenship and Immigration Canada;
  • a notice of decision or temporary resident permit issued by Citizenship and Immigration Canada;
  • a passport; or
  • a citizenship certificate.
