Proof of birth
Acceptable proof of birth can be a photocopy of any of the following documents, on which the child’s family name, given name and date of birth appear:
- a birth certificate;
- a birth registration form certified by the registrar of a province or territory;
- the hospital record of birth;
- the record of the attending physician at the birth;
- the record of the attending nurse or midwife at the birth;
- a record of landing or permanent resident card issued by Citizenship and Immigration Canada;
- a notice of decision or temporary resident permit issued by Citizenship and Immigration Canada;
- a passport; or
- a citizenship certificate.
