Completing the T5006 slip
On each T5006 slip, give the following information:
- Calendar year – This is the year for which the slip is prepared. However, as the reporting period begins on the 61st day of the calendar year and ends on the 60th day of the following year, enter the calendar year in which the reporting period begins.
- Box 1 – Enter the investor's social insurance number (SIN). You have to make a reasonable effort to get the investor's SIN. If you do not, you may be liable to a $100 penalty for each time you do not provide the SIN on a slip. However, when a person tells you that he or she does not have a SIN but is applying for one, make sure you complete and file the return before the filing deadline. If you have not received the recipient's SIN by the time you prepare the T5006 slip, leave box 1 blank.
- Box 2 – If the investor is a qualifying trust, enter the name of the individual annuitant (recipient) under the plan.
- Box 3 – Enter the venture capital corporation's federal registration number.
- Box 4 – Enter the venture capital corporation's account number.
- Box 5 – If the qualifying trust is a spousal or common-law partner RRSP, enter the spouse's or common-law partner's SIN.
- Box 6 – If the qualifying trust is a spousal or common-law partner RRSP, enter the spouse's or common-law partner's name.
- Boxes 7 and 9 – Enter the number of Class A shares that the investor acquired or irrevocably subscribed and paid for within the time specified.
- Boxes 8 and 10 – Enter the consideration paid for the Class A shares that the investor acquired or irrevocably subscribed and paid for within the time specified.
- Investor's name and address – Enter the investor's name and address.
- Corporation's name and address – Enter the labour-sponsored venture capital corporation's name and address.
