As requested, we have reviewed the above mentioned form and provide the following comments for your consideration.
1. For information purposes, you might consider inserting a reference to Information Circular 86-3 in the introduction area at the top of the form.
2. To simplify the form, you might consider changing it so that the form only applies to the 1989 and subsequent taxation years. If so, lines (F) to (K) could be deleted.
3. We have enclosed a mock-up copy of the form which contains the suggested changes.
